School Based Form - This form is used with any in-services done for school-based small or large group meetings.
For posting on SchoolStation prior to the event, complete and fax or send by courier to the Teacher Center.
For posting after the event, attach sign-in sheets and send to the Teacher Center.
Sign-in sheets should be a preprinted school roster. Teachers should sign in beside their printed name. For 6 hour day sessions, provide separate sign-in sheets for the morning and the afternoon.
Evaluations may be provided by the Teacher Center at the administrator's request. Evaluations for school-based sessions are to remain at the school for use in planning continuing in-services.
